Structured Family Caregiving

HomeCourt Home Care is proud to introduce a new program that will help yet another group of people in our community. The Structured Family Caregiving program is designed for those who live with and care for a loved one.

SFC is a Medicaid Waiver program that provides income to those taking on the responsibility of caring for an elderly family member. The caregiver may be family (including spouses) or a friend but must live in the same home as the care recipient. The family caregiver will receive a tax-free stipend for their caregiving commitment to their loved one. The stipend typically ranges from $30.25-$40.79/day.

How do I qualify for this program?

  • The care recipient must be eligible for Medicaid

  • Must need 24-hour, live-in care

  • Needs help with one or more activities of daily living (Such as bathing, toileting, walking, transferring, dressing, eating)

Caregiver Eligibility Requirements:

  • Must live with the participant

  • Can be family or friend

  • May work another job outside of the home, but must agree that the care of their loved one is their top priority

Where does HomeCourt fit in? We help by providing support with regular visits from one of our experienced Caregiver Coaches. We help caregivers feel more confident and supported in their role as their loved one's primary caretaker. This program was created to support and empower family members to provide quality care for their loved one in a way that best meets the needs of their family.

We offer this program to the following counties:

Adams, Allen, Blackford, Delaware, Grant, Huntington, Jay, Wabash, and Wells
